CSA Service Fee Directory Provider Enrollment Form
Dear Provider: Welcome to the Service Fee Directory (SFD). The SFD is a web-based electronic system that has been developed to assist local governments purchasing services funded by the Comprehensive Services Act (CSA) and providers of those services to share information regarding the availability of services and fees for the services. Private and public providers of services list information describing their programs, locations and fees. Interested parties have the ability to locate providers based on either 22 pre-defined services or 24 child characteristics. The SFD lists all providers authorized to receive payment from the Funds Pool except for exemptions listed below:
CRITERIA FOR INCLUSION IN THE SERVICE FEE DIRECTORY:
- Any entity, public or private, to include out-of-state vendors, that provide specialized services for children. Specialized services means a service or activity specifically designed to assist persons with disabilities or at-risk. Note: Any provider, facility, program or service for which licensing standards exist must have in effect a valid license in order to be listed in the directory.
- Individuals not associated with an entity, providing services
- Individually approved foster care families
- Purchase of goods
- Non-specialized services (community activities only indirectly associated with care. e.g. baby-sitting, swimming lessons, transportation, etc)
- Note: Any unregulated provider, facility, program or service for which there are no licensing standards may be included in the SFD. If you desire to post your facility information on the SFD, you must have a USERID and password assigned to you. In order to obtain a USERID and password, please complete the following information and submit. You will be notified via U.S. mail once your USERID and password has been established.
You must complete all fields for this form to process and registration to be completed.
