Hello everyone. It is time for the annual Critical Service Gaps survey as required by the General Assembly. The responses are due by November 30, 2009.

Below you will find a link to a printable PDF document version of this year’s survey for your convenience. Please convene your CPMTs to conduct your annual review of gaps and barriers to community service availability.
