    For those interested in using family satisfaction/family engagement surveys to enhance their programs , two examples are provided. The first is presently being used by a local CSA program.


  • Family Survey Example – longmsw
    The following survey was a joint effort between the Virginia Federation of Families and the Office of Child and Family Services, Virginia Department of Mental Health, Mental Retardation, and Substance Abuse Services. The survey was distributed via e-mail to all family members that have had recent contact with the Virginia Federation of Families and was conducted statewide April 3, 2009-May 1, 2009. The purpose of the survey was to learn about families needs in terms of services and assistance, and also to what extent families are interested in being involved with the Virginia Children’s Services System. The survey template is provided here for local governments that are interested in conducting surveys of their local and/or regional systems to gain the family perspective. You are welcome to use the attached survey in your locality or revise it to fit your needs. Survey results from the statewide survey should be available soon.
